Presentation - Health Promotion Proposal I
This presentation illustrates the health issue: despite the benefits of pulmonary rehabilitation for seniors with COPD, the enrollment rate remains low. I am developing a health promotion proposal and using the health behaviour therapy to try to explain the problem.

In this infographic, I tried to use the "Health Belief Model" to explain the low pulmonary rehabilitation enrollment rate. I suggested to my colleagues the interventions to improve the uptake based on the health promotion model I employed.

Community Engagement
WHO has defined community engagement as “a process of developing relationships that enable stakeholders to work together to address health-related issues and promote well-being to achieve positive health impact and outcomes”. I chose to analyze Diabetes Canada's community engagement plan because of the organization's outstanding contributions to public health, particularly through its commitment to community engagement. Diabetes Canada's initiatives serve as an insightful example of how to mobilize communities for better health outcomes.

Situational Assessment
This situational assessment explores the current challenges and opportunities related to improving access to pulmonary rehabilitation (PR) for seniors living with chronic obstructive pulmonary disease (COPD). COPD is a leading cause of hospital admissions and reduced quality of life among older adults, yet participation in PR programs remains low due to various personal, structural, and environmental barriers. This assessment examines the impact of the current situation, identifies key factors influencing access, and highlights potential actions that can be taken to support more inclusive and effective rehabilitation options for this vulnerable population.

This logic model outlines a community engagement strategy aimed at improving access and participation in pulmonary rehabilitation (PR) for seniors with COPD by addressing key barriers through collaborative, technology-driven, and community-based interventions. Grounded in WHO and NCCDH frameworks, the model integrates multi-sectoral partnerships, digital connectivity, transportation support, and peer engagement to enhance PR accessibility, ultimately leading to better health outcomes and quality of life for seniors.
